Case 1: A Case of Open-Angle Glaucoma Treated With Canaloplasty and Trabeculotomy

A panelist discusses how combining cataract surgery with canaloplasty and trabeculotomy using the Via360 device offered an effective MIGS solution for a patient with open-angle glaucoma and poor medication adherence, reducing IOP while minimizing the need for filtering surgery.

Case 1: Open-Angle Glaucoma Treated With Cataract Surgery and MIGS (Canaloplasty Plus Trabeculotomy)

This case involves a 70-year-old man with open-angle glaucoma and poor medication adherence, leading to progressive visual field loss—most severe in the left eye. Initial medical therapy was complicated by a follicular reaction to brimonidine. Due to noncompliance and disease progression, the patient underwent cataract extraction combined with canaloplasty and trabeculotomy using the Via360 device. This MIGS approach was chosen to avoid filtering surgery risks in a patient with unreliable follow-up. Postoperatively, IOP was successfully reduced, and medication tapering was initiated.
