Case 2: 90-Year-Old Woman with Chronic Angle-Closure Glaucoma and Wet AMD
This case involves a 90-year-old woman with suspected open-angle glaucoma and poorly controlled intraocular pressure (IOP) despite medication (PGA, brinzolamide). Due to confusion with her dosing regimen, the patient had poor adherence. Her right eye exhibited IOP elevation, but the left eye was diagnosed with chronic angle-closure glaucoma, unsuitable for typical MIGS procedures.
After cataract surgery and managing the left eye, the patient underwent Via360 viscodilation for the right eye, focusing on pressurized viscoelastic delivery to enhance outflow. Postoperative results showed a significant IOP drop to 8 mm Hg and improved vision to 20/40, far exceeding expectations. This procedure minimized hyphema risk while providing excellent IOP reduction without the need for goniotomy. The patient’s medication was tapered, and the surgical approach was considered appropriate due to the patient’s complex medication regimen and poor eye drop adherence.
